- Click 'New'
- Add a 'Form Name'
- Add a 'Display Name,' if it is different from the form name
- Create an SEO URL by manually entering it or clicking ‘Auto.' If ‘Auto’ is clicked, the SEO URL will be automatically generated based on the form name
- Click the 'Publish' button to publish the form upon saving
- Click 'Enable CAPTCHA,' if needed
- Enter a confirmation message directly in the HTML or by using the WYSIWYG Editor. This message will be displayed after the user submits the form.
- To send a confirmation email to the form submitter, check the box next to ‘Send Confirmation Email to Submitter,’ then enter the sender information, email subject line and email body text. If you would like the confirmation email to include the information submitted on the form, check the box next to ‘Include Submitted Information’
- To send a submission notification email to a CMS admin, check the box next to ‘Send Notification Email to CMS Administrator(s)’ and enter in the appropriate information in the designated fields
- Enter a custom javascript/CSS, if desired, by checking the javascript/CSS boxes and entering the information
- Click 'Save'
